Service Description
We accept insurance! HIPAA compliant telehealth video consult. Available for use on any PC, laptop, or mobile device. Upon booking, you will receive an email with a link to the patient portal through MilkNotes, which includes all new patient materials and information about your appointment. Payment is due at time of service. If booking online, payment is due at time of booking in order to reserve your time slot. To avoid $75 cancellation fee, 24 hour prior notice is required for all cancellations. For self-pay patients, insurance-coded receipt given at appointment to submit for insurance reimbursement upon request.
