Service Description

HIPAA compliant telehealth phone consult. Upon booking, you will receive an email with a link to the patient portal through MilkNotes, which includes all new patient materials and information about your appointment. Payment is due at time of service. Payment is due at time of appointment via Venmo, Zelle, cash, or check only. To avoid $75 cancellation fee, 24 hour prior notice is required for all cancellations. Telephone consultations do not qualify for insurance reimbursement at this time.
